There are three application forms to be filled.

First is the ETA-750 form

This form is to be filled out by your employer and submitted to the www.dol.gov (That is, Department of Labour website) by your employer.

Second form is the I-140 form

This form should also be filled by your employer and then it should be submitted by your employer to the Immigration and Naturalization Service (INS) of the USA.

Then the third form is the I-485 form

You should fill this form along with all other documents issued to you and your family. It is along with this document that you should fill the EAD i.e, Employment Authorization Document and the Travel Document.

You can have the I-140 and I-485 filled out concurrently. After you have filled the third document which is the I-485, your fingerprints will be taken. Unless your I-140 document is approved, the I-485 document cannot be approved yet.

Once all these forms have been processed and approved by the appropriate agencies, your passport will be stamped and you will be given your plastic green card.
